MAKE AN APPOINTMENT?

Appointments with the doctors, nurses and HCA's may be made by telephoning 290333 between 8.30am and 6.00pm or by calling at the surgery. We have GP appointments each day which are reserved for urgent problems and also appointments which are bookable up to six weeks in advance.

OBTAIN A HOME VISIT?

Home visits are available for patients whose medical condition prevents them from getting to the surgery. You are requested to telephone before 10.00am if a visit is required for that day. Please give the receptionist as much information as possible to enable the doctor to allocate priority to house calls.

Do remember that several patients can be seen at the surgery in the time that it takes to do a home visit.

HOW DO I...
OBTAIN A REPEAT PRESCRIPTION?

Patients on regular medication will require repeat prescriptions. Each time you are issued with a new prescription, a request form is also generated for you to make your next request. Requests cannot be taken over the phone for safety reasons as errors are more likely to occur. Please note the following:

  1. Allow two working days for your prescription to be processed.
  2. If you have more than one repeat prescription, please try to order all your items together.
  3. Clearly indicate on the “repeat prescription request form” where you would like your prescription sent. We can arrange for it to be collected by any of the following chemists.

    Coles Chemist (Darnley Road) Bryant Road Pharmacy
    Williams Chemist (Frindsbury Road) Medimart Chemist (Wells Road)
    Boots (in the High Street) Tesco

    You can also collect your prescription direct from the surgery or we can send it to your home address, though in this instance please remember to include a stamped, self-addressed envelope.
  4. Deliver or post your repeat prescription request form to the surgery.

REGISTER WITH THE PRACTICE?

For information on how to register with the practice, please see a receptionist. All patients are given registration forms and a questionnaire and are asked to bring this when they attend for a new patient health check.

If you have a preference to see a particular doctor or nurse at the practice, please inform the health care assistant/practice nurse at your new patient check. We will do our best to comply with your request.
