PATIENTS' CHARTER

COMMENTS, COMPLAINTS AND SUGGESTIONS

We are constantly trying to improve our services. To help us achieve this, we have a practice-based complaints procedure which is fully approved under the NHS complaints system. However, it is unable to deal with questions of legal liability or compensation. We hope you will use it to allow us to look into and, if necessary, correct any problems you may have identified. Under normal circumstances we will respond to your complaint within 14 days.

If you have any comments, complaints or suggestions, please speak to or write to Mrs Lynne Johnson the practice manager.

PATIENT CONFIDENTIALITY AND DATA PROTECTION

We ask you for personal information so that you can receive appropriate care and treatment. This information is recorded on computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the practice team.

FREEDOM OF INFORMATION - PUBLICATION SCHEME

The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the 'classes' of information the practice intends to routinely make available.

This scheme is available from reception.

PATIENT RESPONSIBILITIES

Don't waste appointments - ring to cancel as soon as possible.

An appointment is for one person only.

Home visits are only for patients whose medical condition prevents them from attending the surgery.

Look after your own health - take exercise and eat healthily.

Remember there are other ways to get help, including NHS Direct and your local pharmacist.

ZERO TOLERANCE

The practice has a zero tolerance policy. If anyone on the premises behaves in a threatening or aggressive manner towards a member of staff, patient or visitor, they will be asked to leave. We reserve the right to involve the police and remove patients from the practice list with immediate effect.
